About the Role
We are seeking a T2A Coordinator to help young people aged 15 to 18 prepare for adulthood, leaving care and the support they may need afterwards.
Working across residential and other non-family-based care, foster and kinship care, and disability services, you will help ensure transition planning starts early and leads to practical outcomes. You’ll work with young people, carers, practitioners and support teams to identify what is going well, where support is needed and what needs to happen next.
This is a coordination and practice guidance role. You will strengthen planning, monitor progress and follow up on gaps while the young person’s care team retains responsibility for day-to-day case work.
You will:
  • Guide transition readiness assessments covering life skills, housing, budgeting, health, identity, education, employment and community connection.
  • Keep each young person’s views, strengths, culture and goals central to their transition plan.
  • Help care teams and carers turn transition goals into practical support and everyday opportunities to build independence.
  • Provide guidance across residential care, foster and kinship care, and disability services.
  • Coordinate with Child Safety, post-care services, NDIS representatives, housing, health and community providers to support timely referrals and follow-up.
  • Support young people to maintain connections with family, kin, culture, Country, community and trusted adults.
  • Identify gaps, barriers and risks, and escalate concerns when needed.
  • Maintain accurate records and monitor progress for young people approaching 18.
 
About You
You understand that a successful transition is about more than a plan on paper. You build trust with young people, listen to what matters to them and work with the people around them to make sure agreed actions happen.
You’ll also be confident providing guidance to care teams and carers, working with external services, managing competing priorities and recognising when a risk or delay needs to be escalated. Experience supporting young people with disability or NDIS-related needs will be valuable.

  • Permanent part-time employment, working 24 hours per week
  • Work pattern which is able to be negotiated with the successful incumbent
  • $60,000 per annum plus superannuation

Requirements
  • A bachelor’s degree in social work, human services, psychology, community services or a related field is desirable. A relevant diploma combined with significant sector experience may also be considered.
  • At least three years’ experience in residential care, transition to adulthood, foster and kinship care, disability services, youth work, case coordination or a related community services setting.
  • Sound understanding of leaving-care pathways and use trauma-informed, culturally responsive and strengths-based approaches in your work.
  • Valid Australian driver’s licence and be able to travel to DCC service locations and community appointments.
  • Satisfactory LCS2/7 outcome
  • Working with Children Check
  • NDIS Worker Screening Check
  • Current First Aid and CPR certification, or willingness to obtain 
  • Satisfactory national police check
